REQUEST FOR CHANGE OF OWNERSHIP TITLE

• Complete a Customer Information Change Form (Name) 
• Provide a Certificate of Payment (COP) from the District Revenue Office, the City Hall or the Borough Corporation.
• Provide two (2) valid forms of Picture Identification (National Identification Card, Driver’s Permit, Passport).
• Payment fee of $95.00 (VAT Inclusive)

If applicant’s information is not registered at the DRO (Changes post 2009), the following additional documents are required

• Original Registered Deed of Property (Copy to be provided)
• Statutory Declaration prepared by a Commissioner of Affidavit (Format available at all Customer Service Centres)
